  For the client it is 1^st person writing structure: “I want Housing” in client own words.

  Your instructor will provide you with a choice of three possible case studies to use as the basis for this project. Select one case study and read it carefully.

  Refer to the Guidelines for Writing a Treatment Plan located on the student website. This document will provide answers to many of your questions and help you organize and structure your paper so that it has a logical flow.

  Write 5 page Treatment Plan Essay assessment and treatment plan on your selected case study. Include the following:

  • In your own words, summarize the client’s history, presenting problem, and symptoms.
  • Show Goals and Objectives (steps toward goals) referrals, etc regarding treatment plan.
  • Identify additional information that you would like to know about the client, and/or additional questions that you would like to ask. Explain why having this information would be helpful to you.
  • Discuss at least two different instruments (treatment plans) and show where you got them from that might be used to further assess the client. Include the name of the instrument or technique that you would use and explain how the results might be helpful in case conceptualization, diagnosis, and treatment planning.
  • Provide a full diagnosis of the client, using the DSM-IV-TR® multiaxial format.
  • Develop a hypothetical treatment plan for the individual based on the data in the case study.
